That the nature of Lucy's outsider-ness is so outwardly un-obvious and so difficult to place is one of the things I find most interesting about her, I think—I love the conflict between her somewhat-real feelings for Dr John and her recognition of that dishonesty, as you say.
I also like the idea that the reciprocated romance with M Paul is actually all fantasy—it certainly wouldn't be beyond Lucy's abilities as an unreliable narrator.
